| Job Description: |
| This position will assist individuals who receive Medical Assistance, Home and Community Based Waiver and Alternative Care services in locating and gaining access to needed waiver and other state plan services, as well as needed medical, social, housing, supportive and other services, regardless of the funding source. This position will coordinate the Community Support Plan/Individual Service Plan; monitoring services so the individual’s health and safety are assured; and conducting reassessments; and a face-to-face visit a minimum of every 3 months for CADI, TBI and EW. |

| Education, training, experience: |
| Requirements for this position include a BSW and a minimum of 2 years experience working with MI clients, preferably with dual-diagnosed individuals (MI/CD, MI/DD). Knowledge of community resources that serves the given population is also required. An LSW or MSW is desired. |
